Clinical Care Managers coordinate care for patients and work with them to control their conditions. Manet’s Diabetes Self-Management Education Program is accredited by the Diabetes Education Accreditation Program of the American Association of Diabetic Educators. Manet also offers a variety of health and wellness programs, such as chronic disease management, healthy eating, falls prevention and tobacco cessation classes, in conjunction with community partners.
To reach an on-call provider after office hours, Manet patients can call 617-376-3000 or their preferred site. For medical emergencies, please call 911 or go to the nearest emergency room.
Manet Community Health Center diabetes self-management education program has been awarded Recognition from the American Diabetes Association (ADA). ADA Recognition is a voluntary process which assures that the approved patient education program has met the national standards set by the ADA. Also, Recognition assures the consumer that the health professionals teaching the program provide the most up to date information about diabetes management. Self-management education is an essential component of diabetes treatment. Manet’s program teaches self-care skills that promote better day to day management and help to reduce acute and chronic complications and hospitalizations. The education program covers topics such as diabetes disease process, nutrition, physical activity, medications, blood sugar monitoring, preventing, detecting and treating complications, goal setting and problem solving and stress management. For more information on this program, or other diabetes or asthma patient education offerings please contact firstname.lastname@example.org.

Narcan/Naloxone, Opioid Overdose and Reversal Program (counseling and distribution)
Narcan at Manet
Narcan is available for anyone to have on hand in case they witness an overdose.
Narcan(Naloxone) is a life-saving medication that can reverse an opioid overdose by blocking opioids receptors from attaching to opioid receptors in the brain. It is administered as a nasal spray. There are NO side effects. Narcan is active for about 30 to 90 minutes in the body—giving enough time to seek emergency medical care.
A Manet Prevention Specialist can teach you how to use Narcan and supply this medication to you free of charge.
