Manet Community Health Center is committed to continuous quality improvement and we encourage patients to take an active role in their care and let us know whenever they have a concern, safety issue or suggestion for improving our services. We also love to hear when we exceed expectations.

    If you feel your patient rights have been violated, you can file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/ocr/privacy/hipaa/complaints/. Or if you have a patient safety concern, you can file a complaint with The Joint Commission Office of Quality and Patient Safety, One Renaissance Boulevard, Oakbrook Terrace, Illinois 60181.
