Manet Community Health Center, Inc. manages your health information and medical records, ensuring your information is maintained confidentially and accordance with applicable state and federal laws and regulations. Please see our Notice of Privacy Practices for more detailed information regarding your rights to your Personal Health Information. If you have further questions or concerns call our main line at 617-376-3000 and ask for Medical Records.

Process for requesting your medical records:

You can view and have access to some of your Personal Health Information using AthenaHealth Patient Portal.

The fastest and easiest way to manage your medical records with Manet Community Health Center, Inc. is to download and complete the Authorization to Share Patient Health Information form.

Use this form to either:

• Obtain records from outside providers by submitting it to your previous provider, or, if needed, dropping it off at your Manet medical office for us to fax on your behalf.
• Send records from Manet to another provider by completing and submitting the form as instructed.
