Manet Community Health Center, Inc. manages your health information and medical records, ensuring your information is maintained confidentially and accordance with applicable state and federal laws and regulations. Please see our Notice of Privacy Practices for more detailed information regarding your rights to your Personal Health Information. If you have further questions or concerns call our main line at 617-376-3000 and ask for Medical Records.

Process for requesting your medical records:

You can view and have access to some of your Personal Health Information using AthenaHealth Patient Portal.

The fastest easiest way to obtain your medical records from Manet Community Health Center, Inc. is to download and print the appropriate form below.

• To obtain records from outside Manet, download here
  (If you are a new patient and need to obtain records from your previous provider please return the completed form to your previous provider, or if you do not have the available resources to do so, you may drop off the form to the Manet medical office that you are currently receiving medical care. We will fax the authorization form for you to obtain your medical records.)
• To send records from Manet, download here
