When accessing your vaccine card on the state’s MyVaxRecord website, please make sure that you have entered the information that you used when you registered for your vaccine.
It’s likely your vaccine record is in the Massachusetts Immunization Information System (MIIS) database, but some of your demographic information may be missing, incomplete, or does not match with the information you are entering to look up your record.
Try these steps to find your record:
Re-enter your information with another version of your name (maiden, a different spelling, etc.)
Re-enter your information with a different email or mobile number
