POSITION TITLE: Site Lead RN
REPORTS TO: Director of Nursing and Clinical Services
General Definition and Scope of Job
Manet Community Health Center is seeking a Senior Lead Site Nurse for its Father Bill Main Spring site. He/she works in collaboration with the Lead Physician and Lead Front Desk staff. He/she will support team development and the efficient clinical care of patients.
Core Responsibilities
Clinical practice:
1. Proactively identify, diagnose and manage treatment plans for patients at risk.
2. Work with patients in order to support adherence to prescribed treatment plans.
3. Assess, plan, coordinate and implement care of patient by phone triage.
4. Implement and participate in vaccination and immunization programs for adults
5. Support patients to adopt health promotion strategies and apply principles of self-care.
6. Manages the assignment for abnormal follow-up to assure complete and safe patient care.
Site management
1. Continually monitor the patient flow of the day and demonstrate flexibility and leadership in supporting the staff so that patients are served efficiently.
2. Together with the front desk lead, provide in-the-moment coordination when provider schedules are altered, as needed.
3. Responsible for the care of the equipment, and completion of environment of care checks.
4. Responsible for the management of safe medication storage.
5. Responsible for ordering supplies and discussion par levels with the Director of Clinical Operations.
6. Responsible for managing the immunizations including out dates, par levels, and safe storage.
7. In collaboration with the Lead Physician and Lead Front Desk staff, facilitate the monthly site meeting. Communicate new policies or procedures to the clinical staff.
8. Act as a resource for Lead nurse in Manet’s Father Bills location
9. Act as preceptor/mentor to new nursing staff.
10. Will support the efforts of the health center to meet federal, state, HMO clinical standards and support efforts to capture the needed data.
11. In collaboration with Senior Management and the Director of Clinical Operations problem solve site issues where appropriate.
Minimum Skills, Experience and Educational Requirements
1. Graduate of an accredited school of nursing from an associates program, bachelors preferred.
2. At least three years’ experience, in ambulatory care or public health preferred but not required.
3. Demonstrated leadership and management skills required. Previous experience supervising staff strongly preferred.
4. Excellent patient care skills required.
5. Ability to handle multiple requests and prioritize appropriately.
6. Strong attention to detail and prompt follow up skills required.
7. Possesses and displays the ability to act independently within the scope of the Nurse Practice Act.
8. Excellent organizational and interpersonal skills required.
9. Communicates courteously, pleasantly and effectively with patients, families, and staff both by telephone and in person, demonstrating superb customer service.
10. Maintains professional composure and confidence during stressful situations.
11. Consistently utilizes good judgment in the resolution of problems, both clinical and non-clinical.
Required Certifications and Licenses
Massachusetts license to practice professional nursing; CPR certificate required.
Critical Demands of the Job
1. Manage multiple requests and prioritize appropriately.
2. Ability to function in a fast-paced setting with a variety of patients and staff.
3. Attention to details and prompt follow up.
4. Includes walking and standing for long periods of time, sitting for short periods of time, hand dexterity, clear hearing and speaking ability. Must be able to lift a maximum of 1/3 of their body weight.
5. Excellent organizational and interpersonal skills.
Working Conditions
1. Works in well-lighted environment with comfortable surroundings.
2. Stressful at times due to competing demands, including attention to multiple patients and staff.
Required Availability
May be required to work a rotating schedule as the department requires. This includes evenings, holidays and weekends.
