General Definition and Scope of Job
The Registered Dietitian with community and patient needs at the forefront provides ongoing leadership and expertise on disease prevention and management programming through comprehensive assessment, planning, implementation and overall evaluation. As a member of the Manet Clinical Team, this Registered Dietitian will be dedicated to the improvement of health outcomes and the reduction of disease state(s) and this will be accomplished through the integration and functions of case management models, health promotion and education, preventive health compliance and wellness opportunities for patients with chronic health conditions, their caretakers as indicated, and community residents. In an effort to produce meaningful behavioral changes, the Registered Dietitian will utilize optimal technology, culturally relevant outreach strategies, provide one-on-one and group counseling and advising, develop and integrated health-related programs and educational and attend and/or facilitate onsite and offsite events, as core to the charge of improving health outcomes for the community. She/he is expected to model a healthy lifestyle, corporate values and high integrity. Moreover, demonstrate leadership and a commitment to innovative approaches to better health and wellness and, functions as a practitioner, consultant, role model, and educator, ensuring quality patient care and outcomes.
Core Responsibilities
- Conducts nutrition assessment and counseling education to individuals, families, and groups. Review results of health screenings and wellness activities with program members, and make effective lifestyle recommendations that lead to meaningful behavioral changes;
- Educates patients, physicians, nurses, educators, and other health care professionals (HCP’s) regarding the importance of intensive management of patients with chronic diseases; Coordinates optimal resources within the health care team of physicians, nurse managers, clinical specialists, patients and families;
- Assists in development of nutrition patient education materials that are culturally sensitive
- Demonstrates confidence and comfort in technology to manage and prevent disease and promote wellness.
- Designs and evaluates chronic disease management related programs including deliver presentations through webinars and workshops;
- Serves as a resource for diabetes care across the continuum of care. To that end, provides comprehensive nutrition assessment of patient with Diabetes including planning, implementation, coordinating, monitoring, and evaluation of a nutrition plan of care;
- Participates in continuous quality improvement activities and educational experience in support of departmental and divisional nutrition philosophy and objectives, as well as Health Center initiatives;
- As a participating diabetes educator instructor, ensures that the national standards for diabetes self-management education are met and maintained at all times; Interprets, implements and enforces the Center’s compliance with JCAHO, IDPH and the Association of Diabetes Care and Education Specialists. Maintains continuing education requirements for ADCES certification. Collaborates with the team to determine outcome measures for the program and new services added. Evaluates program content and effectiveness through the collection of clinical outcome data;
- Maintains a thorough knowledge of applicable Manet policies and procedures; and,
- Other duties as assigned.
Minimum Skills, Experience and Educational Requirements:
- Registered Dietitian-Nutritionist by Commission on Dietetic Registration
- Licensed Dietitian-Nutritionist
- Bachelors in Nutrition
- 5+ years’ experience in direct care or as a telephonic Case Manager/Disease Manager;
- Experience in diabetes self-management education and in teaching individuals and groups;
- Experience in health promotion/wellness a plus;
- Excellent interpersonal and communication skills;
- Able to use Microsoft Office Suite (Word, PowerPoint, Excel), email and the Internet;
- Superior written, interpersonal and oral communication skills ;
- Excellent organizational skills

• Will comply with all applicable laws and will cooperate with the Manet Community Health Center compliance program rules and regulations.
Required Availability
May be required to work a rotating schedule as the department requires. This includes evenings, holidays and weekends.
