Welcome
New Patients!
We
are pleased that you have selected us as your Primary Care Provider!
Our
Family Practice Physicians, Nurse Practitioners and Physician Assistants
are looking forward to working with you as a team to keep you healthy.
Your Primary Care Provider will see you for almost all your health
care needs. This includes preventive health visits, routine check-ups,
and follow-up for chronic illnesses. (Click here
for a complete list of programs and services available to you at
our centers.)
Your
Primary Care Provider or one of the covering providers at our health
centers will see you for immunizations (shots), pap tests, scheduling
your mammograms, as well as be available to you if you suddenly
become ill. We want to assure you that we can be reached 24 hours
a day, seven days per week through our main phone numbers (click
here for contact information for
each of our sites). Should you have a life threatening emergency,
however, go immediately to the nearest hospital. If you have a question
about whether or not you should go to the hospital, please call
us to guide you. If you are unable to call us before going to the
hospital or emergency room, please call us within 24 hours so we
can assist you with your health care needs.
Our
doctors are on staff at Quincy Medical Center. Should you ever require
admission to a hospital, our doctors can admit you there and care
for you in the hospital. Our doctors work closely with the specialists
at Quincy Medical Center. If it becomes necessary for your to see
a specialist, our providers will coordinate your visit to a specialist
with whom they have an established relationship. Most of these specialists
are at Quincy Medical Center. (Click here
for more information about referrals on the Billing and Insurance
page.)
If
you have any questions or concerns, please call the main number
at your health center and one of our staff will be happy to help
you (click here for contact information
for each of our sites).
