Preparing
for Your Visit
- Visit
Coverage: You will need to bring your Insurance, Health Coverage Plan, or Health Safety Net (HSN) card with you for every visit. If you do not
have medical coverage or are unable to pay your deductibles and
co-insurance, bring proof of your income and residency and we
will assist you in trying to find a program to fit your needs.
- Medications:
You should bring all the medications you are taking with you on
your appointment. Bring the actual containers of the medications
or a list of the medications, including the dosage and the number
of times you take it per day. Include over-the-counter medications.
- List
of Concerns: Prior to your visit, write a list of the issues that
you have and would like to discuss with your health care provider.
- Family/Friend:
It may be helpful to bring a family member or friend with you
to the office.
- Interpretation: Certified Medical interpreters are available in different languages. Please let us know of your language preferences.

at the Office
Check in at the registration area when you arrive.
Beginning
the Exam
Blood pressure and weight measurements are taken at every visit.
They are good screening tests and important to have in order to
prescribe certain medicines and to make good judgments about your
care.
During
the Visit
There are several types of visits you may encounter:
- Physical
Exam: This type of visit involves a longer, more detailed exam.
If you need a physical exam, be sure to schedule it weeks in advance.
You should have a physical exam each year.
- Acute
Problem: The focus of this type of visit is to address a specific
concern or illness.
- Several
Problems: There may not be time to address all your concerns.
It may help to rank them in order of importance so that the most
concerning issues can be dealt with first.
Ask
Questions!
If
at any time during or after the visit you do not understand something,
make sure to ask us questions. Our health care providers and nursing
staff are available to answer your questions. It is important for
you to understand and be informed about issues surrounding your
health.
After
the Visit
Your health care provider will tell you if you need a lab visit
or other diagnostic test like an x-ray. He or she will also tell
you when you should return to the health center for your next follow-up
visit.
Ask
if your lab tests are being done at the health center or if they
are being sent out to another lab. Most of our lab tests are sent
to Quincy Medical Center or Quest. If your lab test is being sent
out, that facility will also need your Insurance, CenterCare, or
Free Care information and a copy of your card. If they do not have
your medical coverage information, they will send you a bill for
the total amount.
